Before we can see you at The Wellborn Clinic, you must complete the necessary forms for the type of weight loss surgery you are considering. All patient packets include Medical History, Physical and Patient Profile forms. Please download and print the appropriate patient packet below and complete each form before coming to your scheduled appointment. You may download the Post-Op Instructions and Gastric Bypass Patient Labs as needed.
